Direct Deposit
This form is used to request to deposit distributions directly into the registered owner’s checking or savings account at the financial institution.

This form is used whenever the distributions from Cornerstone Realty Fund are to be sent to a name and address other than the registered owner.

This form may be used by any current investor to change the Investor’s address for delivery of information and distributions (if applicable).

This form is used to transfer a number of shares to a specified transferee.

Change of B/D or Advisor Form
This form may be used by any current investor or Registered Representative to change the Broker Dealer or Advisor on an account.
